Home care nurses have an opportunity to profoundly affect the lives of their patients. Providing care and services that allow the patient to stay in the comfort of their own homes creates a significant, positive impact on their quality of life.
Our nursing staff works collaboratively with an interdisciplinary team that can include pharmacists, physical, occupational, and/or speech therapists, medical social work, chaplain services, and others to provide care and services to patients in the home or alternate site setting.
Working independently, they are the eyes and ears for the physician, managing the plan of care and providing updates and clinical assessments with suggestions for modifications as needed.
