At Duke Medicine, we believe the essence of nursing lies of the interaction between a nurse and their patient. And nothing is more important to us, as a system or as individuals, than providing excellent service to each and every patient and family in our care.
That’s why we established the Duke Clinical Ladder program - to honor the expertise of nurses who make a difference at the bedside and, and recognize those who show leadership within their unit, department and institution, as well as on regional and/or national levels.
The Clinical Ladder program at Duke has three distinct tracks: Education, Practice and Management.
The Clinical Ladder program is completely self-selected. Participation is not mandatory, nor is there a limit on the number of nurses who can participate in the program. All RNs with at least two years of experience, and one year of full-time service at Duke may apply.
Eligibility requirements and rewards for nurses at various levels are as follows:
Clinical Nurse I (CNI)
- Nurses with less than six months experience are eligible
- Compensation changes annually
Clinical Nurse II (CNII)
- Nurses with 6 – 12 months experience
- Completed application required for program participation
- Compensation changes annually
Clinical Nurse III (CNIII)
- Nurses with more than five years experience are eligible
- Completed application required for program participation
- 5% increase
- Administrative time = 52 hours
Clinical Nurse IV (CNIV)

A variety of accomplishments and activities, from patient education and participation in community health programs to presentations at professional conferences, are factored into considering those recognized under the program.
