 Occ Summary

Responsible for coordinating continuum of care a ctivities for assignedpatients and ensuring optimum utilization of resou rces, servicedelivery and compliance with external review agencies' requ irements.

 

Work Performed

 

Coordinate and facilitate t imely implementation of discharge plans forassigned patients; arrange fo llow-up care as appropriate.Maintaineffective communication with health care team members related todischarge planning and utilization managemen t.Documentfindings,discharge arrangements and actions taken according t odepartmental guidelines; prepare reports and maintain records asrequire d.Act as an educational resource and provideconsultation to hospital med ical personnel regarding discharge planningprocess and applicable federa l, state and local regulations; identifybenefits, implications and limit ations of home care as appropriate.Provide concurrent review and follow up with appropriatepersonnel regarding denials.Collect denial data fortrendingWork with appropriate staff to develop processes toprevent denials and improve reimbursement.Collect analyze andreport data for resource u tilization, compliance, QI and other areas asassigned.Provide feedback t hat will enhance negotiations withpayers.Develop and maintain positive r elationships withcustomers internal and external to Duke Health System.P erformother related duties incidental to the work described herein.

BSN or MSW required

 

Experience

 

3 years of relevant experience

 

Degrees, Licensures, Certifications

 

Requires ACM or CCM certification within 3 years of hire. BSN must have current or compact RN licensure in the state of North Carolina. MSW must have a current North Carolina license as a CMSW within 90 days of hire or a current license as an LCSW.
